The ICF/MR facilities serve individuals ages 3 and up while the off-site housing is specific to individuals age 18 years and up and who are enrolled in the CAP I/DD program. ICF/MR facilities provide 24/7 supervision and nursing services. Individuals living in the ICF/MR residences attend school either at the Tammy Lynn Center or in the community and participate in activities both on and off campus. The child care subsidy is based on an assessment of need, income and family size. If you are eligible your case manager will assist you in finding a program that meets the needs of your child/children. Parental choice is honored whenever possible. Parent fee's are assessed for families who are income-eligible. The amount paid for each child depends on:
- your family situation
- number of members in family
- your gross income (gross income guidelines for children with special needs)
- the cost of childcare provided.
Each county receives an annual allocation of state and federal funds used for subsidized child care services for its residents. Wake County also receives funding from the Wake County Smart Start Program. We provide information about choosing and using quality childcare. Local money for childcare scholarships is received from the Wake For Kids Fund, which is provided by the Let's Get To Work Program.

CC4C services will be provided in Wake County by a CC4C Care Manager (nurse or social worker) through Wake County Human Services. North Carolina families residing outside of Wake County should contact their local health department.
CC4C Care Managers will assess current and ongoing medical and family needs and concerns. Concerns and services may include, but are not limited to: developmental delay, disability, chronic illness, social and emotional stress and disorders, periodic medical, behaviorial, developmental and psychosocial evaluations.The CC4C Care Managers will establish a plan of care and a schedule of contacts with the family based on the child's individual situation. The CC4C Care Manager will be working closely with the child's doctor to make sure the family is able to keep medical appointments and to follow the doctor's medical advice.

| Match score: 100 | | The Raleigh CDSA provides service coordination, evaluations, and intervention (through a network of local service providers)for children ages birth to three who are at risk for or have developmental delays and meet eligibility criteria of the North Carolina Infant Toddler Program, under the Individual's with Disabilities Education Act (IDEA). Referrals can be made by parents, physicians, Child Service Coordinators, therapists, or other community agencies.
