General information about services/agency: | Residential services are provided in 3 on-site ICF/MR facilities housing 10 individuals each and in 3 off-site group homes housing 4 CAP I/DD individuals each. The ICF/MR facilities serve individuals ages 3 and up while the off-site housing is specific to individuals age 18 years and up and who are enrolled in the CAP I/DD program. ICF/MR facilities provide 24/7 supervision and nursing services. Individuals living in the ICF/MR residences attend school either at the Tammy Lynn Center or in the community and participate in activities both on and off campus. Individuals living in the off-site housing participate in Day Activities at the Tammy Lynn Center and in activities in the community. Off-site housing provides 24/7 staff supervision. |
