The Medical Home is not a place or a program. It is a system of care in which children with special health care needs can receive services that are accessible, family-centered, continuous, comprehensive, coordinated, compassionate, and culturally-competent.
These two websites offer educational materials, guidelines for care, evaluation tools, and state and local resources for the medical home. Providers can use this information to improve the services they offer while parents/advocates can use it to remind providers what they should be trying to accomplish.
